Annual Permission Slip
The annual permission slip is used for most day trips taken by the youth group.
In some cases a trip may require a specific form for that trip. In that event, we will
try to have the form posted here for download. Any overnight trip requires a form
specifically for that trip, and those will be posted in this section.
