You can print this permission slip out to use with any trip that the children take. (Unless a specific form is required by the location)

Annual Permission Slip

This form can be filled out once, to cover most day trips taken by the group.  In the event of an overnight trip an individual form is required.
We will try to get a copy of that specific form posted here as needed.
